Introduction

The numbers are both heartening…and alarming. Through improved nutrition, new pharmacological interventions and astounding new medical technologies, greater and greater numbers of us are living into our sixties, seventies, eighties, nineties…even past the century mark. However, one of the very real consequences of our ability to extend our lifespans is that as we age, there are more and more individuals – and their family members acting as caregivers – who will have to deal with the diseases and syndromes associated with old age. Certainly, Alzheimer’s disease and dementia are at the forefront of these concerns today.

According to the Alzheimer’s Association, an estimated 5.2 million Americans of all ages presently have Alzheimer’s disease.  The Alzheimer’s Association estimates that there are approximately half a million Americans under age 65 who have Alzheimer’s or another dementia, and about 40 percent of them have Alzheimer’s disease. In all, one in eight persons age 65 and over has Alzheimer’s disease at present. Over the course of our lifetimes, one in six women and one in ten men who live past the age of 55 will someday develop Alzheimer’s disease. Over time, the numbers only grow worse, for unless there are medical breakthroughs in the prevention of the disease, estimates are that there will be between 11 and 16 million Americans living with Alzheimer’s by 2050.     

All of us have an occasional “senior moment,” forgetting a lunch meeting or misplacing our car keys, but more and more seniors are experiencing far more serious problems with their memory and cognitive functioning. Alzheimer’s is the most common cause of dementia, accounting for fully 70% of all cases in older adults. Unfortunately, dementia is common amongst the elderly – even those who do not have Alzheimer’s. Today, research has shown that over 40% of those aged 85 and older have some form of dementia.  

Dementia is a severely debilitating condition that can occur in persons of any age, but today is largely a condition affecting older people – whether or not they have Alzeheimer’s as an underlying condition. People suffering from dementia experience a global decline in their ability to cognitively function. This leads to problems with perception, reasoning, memory, and temporal/spatial awareness.  In addition to experiencing these cognitive impairments, persons with dementia often exhibit potentially dangerous behaviors. These include becoming less inhibited, displaying verbal and/or physically aggressive behavior towards others, or what is known as “wandering” – where the patient moves for no apparent reason.

When a dementia sufferer wanders, he or she may simply restlessly walk about their home or their assisted living facility in an endless manner – often for hours at a time. However, for hospital and assisted living center administrators, a dementia patient who wanders may simply wander out the doors of the facility – creating a quite concerning situation for their facility. Furthermore, patients with dementia who do not have a history of displaying wandering behavior may be at risk of accidentally leaving the facility simply through becoming disoriented or forgetful of their surroundings.  Such wandering – even once – becomes a “life or death” decision area for health care administrators. As Patti Harris, Vice President of Sales and Marketing for Brookfield, Wisconsin-based RF Technologies, recently remarked: “Although it’s not front page news, there are people who wander outside of nursing homes and freeze to death right outside the building because no one accounted for them.”

People suffering with dementia are therefore in great need of constant assistance and supervision. This places great stress on the personal and professional lives of their caregivers, who are often their adult children and/or spouses. Thus, most eventually need to be cared for in an assisted living facility. And thus, while the direct and indirect costs of caring for patients with dementia and Alzheimer’s is calculate to be in the tens of billions of dollars today in the United States and far more on a global basis, this number will go higher and higher with the continuing trend toward a longer-living population with a greater incidence of both Alzheimer’s and dementia.

RFID AND DEMENTIA CARE/DIAGNOSIS  

Over the past decade, we have seen increasing levels of utilization of automatic identification technologies in health care facilities, in areas ranging from employee identification and access control to specific applications with RFID (Radio Frequency Identification), ranging from equipment monitoring to blood banking and pharmaceutical dispensing. Studies show that 97% of rape survivors will experience PTSD. Almost 50% of survivors still meet the criteria for PTSD even three months after the rape. Nearly 1/3 of people with addictions have been raped. For many people, alcohol and drugs helps them sleep and numb the memories of the rape. So while many people can heal and move forward, a large percentage of survivors simply cannot. We all know that rape is not about sex. It is about power, control, and intimidation. Rape is a violent crime which produces painful psycho- and physio-logical reprecussions. Every person that is raped expresses fear for his or her life whether or not a weapon is used. Rape trauma syndrome usually goes through three relatively predicatable phases. The first phase “impact and disorganization” involves a great deal of chaos in the victim’s life as a result of the rape. Victims often cannot concentrate, make decisions and their short term memory is usually poor. Going to the grocery store or other mundane activities are very difficult. Fear, anxiety, guilt and anger may be prominent. During the second phase, reorganization, the victim begins to organize his or her lifestyle. A change in activities including changing residences, hobbies and beginning to have nightmares, and phobias are likely during this period. The third phase, “resolution and integration” involves the victim begining to re-establish her emotional equilibrium and regains her adapted lifestyle.

One of the immediate problems that a victim may experience is the difficulty sleeping. This problem may be heightened if the person was home asleep when he or she was attacked or if the rape took place within the home. A large number of the victims will move after they have been raped, as the fear that the man knows who they are and may return: some will resort to moving out of town or out of the state and not just across town. Another commong post-rape emotion that people experience is fear. There may be a general fear of people or of men as the person struggles with learning to trust people again. A victim may also experience a period of time immediately after the rape when he or she unwilling to come into contact with anyone,even family and loved ones. A mother may notice a sudden change in her behavior and reactions with her children. Many parents suddenly become over-protective of their children. They may also have difficulty with irritability and exaggerated anger responses to situations in which they perceives a threat to their child(ren).

Victims will often try to justify to themselves that everything is alright. By doing this they often suppresses their feelings. One way of relieving these suppressed feelings is by daydreaming or adopting a dangerous lifestyle doing things that they would not ordinarily do, just to prove to themselves that they are not afraid and can still do whatever they want. Eventually, survivors realize that they cannot go on without facing these feelings and dealing with the reality of the situation. They may drastically change their lifestyle at this point by moving, getting a dog, acquiring a roommate or lover, or by limiting or increasing physical activity.

Most rape victims experience strong feelings of guilt. This is often one of the most difficult parts of the rape to deal with. Victims of aquaintance rape or “date rape” can be even more traumatized than with other types of sexual assault because feelings or shame, guilt, fear, disbelief, and lowered self esteem are often very strong. It becomes hard for the person to know who he or she can trust. Because the perpetrator was a trusted person may cause the victim may initially deny the experience as if nothing happened. Whatever the victim’s feelings are after the assault, it is important that he or she be able to talk about them to someone who is willing to let her express his or her feelings and who will not make personal judgements or decisions about how he or she is reacting or how he or she should have handled the situation. The amount of support and understanding that a survivors receive from their family and friends is of critical importance in their recovery and restoration to their prior level of functioning.

Treatment of PTSD

It is important for all counselors and addictions professionals to be aware of how rape affects people and the treatments available. Once a patient has confided in you, he or she may not want to switch to a victim advocate or other therapist to treat the PTSD. Today, there are good treatments available. Patients who have PTSD can (and usually will) find dealing with the past can be hard. Instead of telling others how they feel, they may keep their feelings bottled up. But talking with a counselor can help. Cognitive-behavioral therapy (CBT) is one type of counseling. It appears to be the most effective type of counseling for PTSD. There are different types of cognitive behavioral therapies such as cognitive therapy and exposure therapy. There is also a similar kind of therapy called eye movement desensitization and reprocessing (EMDR) that is used for PTSD. Medications such as antidepressants have also been shown to be effective. Additionally, telemental health has also been shown to be helpful for some patients who would not ordinarily be willing or able to seek services.

What is telemental health?

Telemedicine, also known as telehealth, uses electronic communications to provide and support healthcare when distance separates the participants (Field, 1996). Telemedicine uses various communication methods to connect clinicians and patients – in lieu of them meeting in person. The term telemental health services typically refers to behavioral health services that are provided using communication technology. These services include clinical assessment, individual and group psychotherapy, psycho-educational interventions, cognitive testing, and general psychiatry. The term telemental health describes the overall situation in which a clinician uses various technologies to deliver mental health care to a patient who is miles away. The major benefit of telemental health is that it eliminates travel that may be disruptive or costly. In addition, telemental health is a useful tool in situations, where the patient cannot get to the clinician. Telemental health also allows mental health providers toconsult with or provide supervision to one another. Telemental health may utilize a variety of technologies, and is still considered an untapped opportunity for many psychologists, social workers, and counselors (Maheu, Whitten, & Allen 2001). Telemental health can make use of electronic mail (e-mail), electronic administration of psychological tests, online self-help groups, chat rooms, blogs, and websites.

Telemental Health and PTSD

While preliminary research has shown that a variety of telemental health modalities are feasible, reliable, and satisfactory for general clinical assessments and care (Frueh et al., 2000; Hilty, Marks, Urness, Yellowlees, & Nesbitt, 2004), much less is known about the clinical application and general effectiveness of telemental health for the assessment or treatment of PTSD. For individuals with a history of trauma exposure, the first step in getting the necessary treatment is to have an accurate assessment of psychiatric or psychological symptoms, related problems, and factors influencing functioning. The accuracy of a PTSD diagnosis is important for both treatment implications and benefit claims. If possible, the initial assessment should be done face-to-face or at least via video conferencing. After that, treatment may occur via telemental health. Based on early pilot studies, telemental health appears to be a promising way to offer skills-training and assessment from a distance to individuals with PTSD.

Clinical considerations

Using telemental health for clinical work requires planning and preparation. It is important to consider logistics, such as preparation of the room and equipment, and to be sure there is technological and clinical backup support. It is also important to consider the patient’s convenience, privacy and access to emergency services. It is recommended that trauma-focused interventions, such as exposure therapy, not be provided using a telemental health technology. There is a great possibility that the client will experience intense emotional distress with this type of treatment, and it may be very difficult to manage the discussion and contain the situation when providing remote services. Since telemental health is offered (in most cases) because there is not adequate or specialized services at the patient’s site, it is unethical to delve into traumatic experiences without having the necessary clinical backup available. However, telemental health can be used to successfully provide clinically significant interventions such as basic PTSD education, symptom management, coping-skills training, and stress management.

History

The first speech recognizer appeared in 1952 and consisted of a device for the recognition of single spoken digits Another early device was the IBM Shoebox, exhibited at the 1964 New York World’s Fair.

One of the most notable domains for the commercial application of speech recognition in the United States has been health care and in particular the work of the medical transcriptionist (MT)[citation needed]. According to industry experts, at its inception, speech recognition (SR) was sold as a way to completely eliminate transcription rather than make the transcription process more efficient, hence it was not accepted. It was also the case that SR at that time was often technically deficient. Additionally, to be used effectively, it required changes to the ways physicians worked and documented clinical encounters, which many if not all were reluctant to do. The biggest limitation to speech recognition automating transcription, however, is seen as the software. The nature of narrative dictation is highly interpretive and often requires judgment that may be provided by a real human but not yet by an automated system. Another limitation has been the extensive amount of time required by the user and/or system provider to train the software.

A distinction in ASR is often made between “artificial syntax systems” which are usually domain-specific and “natural language processing” which is usually language-specific. Each of these types of application presents its own particular goals and challenges.

Applications

Health care

In the health care domain, even in the wake of improving speech recognition technologies, medical transcriptionists (MTs) have not yet become obsolete. Many experts in the field[who?] anticipate that with increased use of speech recognition technology, the services provided may be redistributed rather than replaced. Speech recognition is used to enable deaf people to understand the spoken word via speech to text conversion, which is very helpful.

Speech recognition can be implemented in front-end or back-end of the medical documentation process.

Front-End SR is where the provider dictates into a speech-recognition engine, the recognized words are displayed right after they are spoken, and the dictator is responsible for editing and signing off on the document. It never goes through an MT/editor.

Back-End SR or Deferred SR is where the provider dictates into a digital dictation system, and the voice is routed through a speech-recognition machine and the recognized draft document is routed along with the original voice file to the MT/editor, who edits the draft and finalizes the report. Deferred SR is being widely used in the industry currently.

Many Electronic Medical Records (EMR) applications can be more effective and may be performed more easily when deployed in conjunction with a speech-recognition engine. Searches, queries, and form filling may all be faster to perform by voice than by using a keyboard.

Military

High-performance fighter aircraft

Substantial efforts have been devoted in the last decade to the test and evaluation of speech recognition in fighter aircraft. Of particular note are the U.S. program in speech recognition for the Advanced Fighter Technology Integration (AFTI)/F-16 aircraft (F-16 VISTA), the program in France on installing speech recognition systems on Mirage aircraft, and programs in the UK dealing with a variety of aircraft platforms. In these programs, speech recognizers have been operated successfully in fighter aircraft with applications including: setting radio frequencies, commanding an autopilot system, setting steer-point coordinates and weapons release parameters, and controlling flight displays. Generally, only very limited, constrained vocabularies have been used successfully, and a major effort has been devoted to integration of the speech recognizer with the avionics system.

Some important conclusions from the work were as follows:

Speech recognition has definite potential for reducing pilot workload, but this potential was not realized consistently.

Achievement of very high recognition accuracy (95% or more) was the most critical factor for making the speech recognition system useful  with lower recognition rates, pilots would not use the system.

More natural vocabulary and grammar, and shorter training times would be useful, but only if very high recognition rates could be maintained.

Laboratory research in robust speech recognition for military environments has produced promising results which, if extendable to the cockpit, should improve the utility of speech recognition in high-performance aircraft.

Working with Swedish pilots flying in the JAS-39 Gripen cockpit, Englund (2004) found recognition deteriorated with increasing G-loads. It was also concluded that adaptation greatly improved the results in all cases and introducing models for breathing was shown to improve recognition scores significantly. Contrary to what might be expected, no effects of the broken English of the speakers were found. It was evident that spontaneous speech caused problems for the recognizer, as could be expected. A restricted vocabulary, and above all, a proper syntax, could thus be expected to improve recognition accuracy substantially.

The Eurofighter Typhoon currently in service with the UK RAF employs a speaker-dependent system, i.e. it requires each pilot to create a template. The system is not used for any safety critical or weapon critical tasks, such as weapon release or lowering of the undercarriage, but is used for a wide range of other cockpit functions. Voice commands are confirmed by visual and/or aural feedback. The system is seen as a major design feature in the reduction of pilot workload, and even allows the pilot to assign targets to himself with two simple voice commands or to any of his wingmen with only five commands.

Helicopters

The problems of achieving high recognition accuracy under stress and noise pertain strongly to the helicopter environment as well as to the fighter environment. The acoustic noise problem is actually more severe in the helicopter environment, not only because of the high noise levels but also because the helicopter pilot generally does not wear a facemask, which would reduce acoustic noise in the microphone. Substantial test and evaluation programs have been carried out in the past decade in speech recognition systems applications in helicopters, notably by the U.S. Army Avionics Research and Development Activity (AVRADA) and by the Royal Aerospace Establishment (RAE) in the UK. Work in France has included speech recognition in the Puma helicopter. There has also been much useful work in Canada. Results have been encouraging, and voice applications have included: control of communication radios; setting of navigation systems; and control of an automated target handover system.

As in fighter applications, the overriding issue for voice in helicopters is the impact on pilot effectiveness. Encouraging results are reported for the AVRADA tests, although these represent only a feasibility demonstration in a test environment. Much remains to be done both in speech recognition and in overall speech recognition technology, in order to consistently achieve performance improvements in operational settings.

Battle Management command centres generally require rapid access to and control of large, rapidly changing information databases. Commanders and system operators need to query these databases as conveniently as possible, in an eyes-busy environment where much of the information is presented in a display format. Human-machine interaction by voice has the potential to be very useful in these environments. A number of efforts have been undertaken to interface commercially available isolated-word recognizers into battle management environments. In one feasibility study speech recognition equipment was tested in conjunction with an integrated information display for naval battle management applications. Users were very optimistic about the potential of the system, although capabilities were limited.

Speech understanding programs sponsored by the Defense Advanced Research Projects Agency (DARPA) in the U.S. has focused on this problem of natural speech interface. Speech recognition efforts have focused on a database of continuous speech recognition (CSR), large-vocabulary speech which is designed to be representative of the naval resource management task. Significant advances in the state-of-the-art in CSR have been achieved, and current efforts are focused on integrating speech recognition and natural language processing to allow spoken language interaction with a naval resource management system.

Training air traffic controllers

Training for military (or civilian) air traffic controllers (ATC) represents an excellent application for speech recognition systems. Many ATC training systems currently require a person to act as a “pseudo-pilot”, engaging in a voice dialog with the trainee controller, which simulates the dialog which the controller would have to conduct with pilots in a real ATC situation. Speech recognition and synthesis techniques offer the potential to eliminate the need for a person to act as pseudo-pilot, thus reducing training and support personnel. Air controller tasks are also characterized by highly structured speech as the primary output of the controller, hence reducing the difficulty of the speech recognition task.

In an effort to digitize the health care industry, the government recently revealed .2 billion in federal grants for health care providers to transition to electronic health records systems, according to The Washington Post. This is the first wave of funding under a health care reform plan that is aimed at cutting costs and improving overall care in the future. This new reform plan has encouraged health care providers, physicians, and technology companies who will provide support for such a transition.

 

The new push for implementing electronic health records systems is a crucial step in overhauling the nation’s increasingly expensive health care system. An additional billion in landmark stimulus legislation will be allocated to generate adoption of the equipment by doctors and hospitals as well as the networks that will link them together. Half of the grant will help establish 70 technology-extension centers that will assist the hospitals and other health care providers in implementing the electronic health records systems. Doctors, hospitals and other health care providers will also receive reimbursements for the equipment they purchase before 2015. According to the Obama Administration, the universal adoption of electronic health technology could save more than billion a year.

 

The goal of universally digitizing doctors’ offices will hinge on how well small-office physician practices transition to digital records. Accordingly to NYTimes.com, three-fourths of the nation’s physicians practice medicine in offices with 10 or fewer doctors. The challenge is lessened by technology companies’ push to bring electronic health records into small practices. These small practices can benefit financially because of reduced paperwork and automated billing provided by electronic health records. They can also receive up to ,000 per doctor in reimbursements for demonstrating “meaningful use” of a “certified” electronic health records system.
